Transition Classes for Adolescents with Diabetes
Adolescence is a time of transition from childhood to adulthood. This process can be an exciting and often trying time for all involved.
The Children's Hospital of Wisconsin Diabetes team wants to assist families in this transition. Our focus will be on helping our patients assume responsibility for their health over time.
Classes may be scheduled during clinic visit or by calling the Diabetes Clinic at (414) 266-2861.Note: Due to the sensitive topics that will be discussed during class, parents and teens will be separated. We do discourage bringing younger siblings (12 and under).

Transition 1 class is an interactive class for those patients who have completed 7th grade to sophomore year of high school. Transition 2 is the final class and is geared toward juniors, seniors and college freshman or sophomores. This is an interactive class with group discussions.
