Memorial and honor gifts
There are many ways to mark a milestone. Whether you want to memorialize a loved one or recognize a special life moment, your gift supports Children’s Hospital of Wisconsin and the care we provide while marking a momentous occasion in a meaningful way.
Memorial gifts may be made in remembrance of a loved one through a donation in their name. Your meaningful and generous gift will support Children’s, and the family of the named individual will receive acknowledgement of your gift.
Memorials with gifts surpassing $1,000 also have the option to be included on a special display in our hospital lobby. The family of the named individual will be contacted about this opportunity.
If you have received gifts in memory of a loved one that are designated for Children's Hospital of Wisconsin, please forward them to:
Children's Hospital of Wisconsin Foundation
PO Box 1997, MS 3050
Milwaukee, WI 53201
Honor gifts can help mark a loved one’s milestone in a thoughtful way. You may choose to honor an occasion such as a birthday, wedding, bar/bat mitzvah, anniversary or a remission date with a gift to Children’s Hospital of Wisconsin. Once the gift is made, honorees will be notified of the donor’s generosity through a personalized acknowledgment card.
Make your gift online or by mail
You can make an honor or memorial gift online—use the appropriate checkbox after selecting your gift amount. Or, send your gift by mail using our printable donation form, and please be sure to include the honoree name and the notification recipient name and address so that we can send an acknowledgment of your gift.
