Learn about privacy practices
Children's Hospital of Wisconsin’s employee health insurance plans comply with the Health Insurance Portability and Accountability Act. If you are covered by one of these plans, you may obtain a copy of your health plan's HIPAA Notice of Privacy Practices from our intranet. This notice describes your insurer's responsibilities and practices in regard to your protected health information.
A copy of each plan's Notice of Privacy Practices is available to all plan participants. The notice is posted on the intranet under Human Resources/Benefits/View Your Lifestyle Benefit Booklet. Select your entity and click to view Legal Notices. A paper copy can be requested by calling Human Resources at (414) 266-3130 or toll-free at (800) 766-6336.
The employee health insurance plans provide health benefits to eligible employees and their eligible dependents as described in the summary plan description for each plan. The plan creates, receives, uses, maintains and discloses health information about participating employees and dependents in the course of providing these health benefits.
Each plan is required by law to provide notice to participants of its duties and privacy practices with respect to protected health information. Our plans meet these requirements by providing a Notice of Privacy Practices, which describes the ways the plan uses and discloses protected health information.
