Institutional Review Board

The Institutional Review Board of Children's Hospital of Wisconsin reviews all proposed research projects that involve human subjects to assure that the rights of such subjects are protected, that adequate and informed consent for their participation is obtained, and that any possible benefits of the research are commensurate with the risks involved.  The chief objectives of every IRB protocol review are to assess the ethics of the research and its methods, to promote fully informed and voluntary participation by prospective subjects who are themselves capable of making such choices (or, if that is not possible, informed permission given by a suitable proxy) and to maximize the safety of subjects once they are enrolled in the project.

March 15, 2012 

New HIPAA forms for research: As of March 1, 2012 the HIPPA forms to be used with protocol submissions to the IRB have been updated.  These forms are currently available for download from IRBnet (www.irbnet.org) and will be available on the CHW IRB website shortly.   These new forms should be used for all new protocol submission and with amendments that required updates to HIPAA forms.  Please be sure to use the new forms with all future IRB submissions.  If you have question regarding how to use the forms or how to complete them please contact Diane Bauer in Corporate Compliance (dbauer@chw.org or 414-266-6237).   

Updated Contact for research involving Radiology Services: Marjorie Bessette is now the contact for all protocols that may involve radiology services for research.  Please contact Marjorie early on in your protocol development to confirm that any radiology services you may require can be accommodated.   Protocols can be shared with Marjorie in IRBnet or she can be contacted at MBessette@chw.org  or 266-3484.
