e-Consult Process
Below is a brief description of the e-Consult process. If you have further questions please call (414) 266-3456 or e-mail csg@chw.org.
Step 1: Once an e-Consult form is submitted, the Clinical Navigation Team will review the e-Consult request. The lead Clinical Navigator (pediatric hospitalists) will either call or e-mail the submitting provider with details on expectations, next step and contact information. (24-48 business hours).
Step 2: The Clinical Navigator will determined whether a single or multiple specialties are needed to complete the e-Consult. If a single specialty is necessary, the team will hand-off the e-Consult to the specialty e-Consult physician leader. If the e-Consult requires more than one specialty, the Clinical Navigator will form a team with the needed specialty e-Consult physician leaders to review the e-Consult. (24-72 business hours).
Step 3: Patient release forms and medical records are gathered by the specialty physician or the Clinical Navigation Team.
Step 4: The specialty physician or the Clinical Navigation Team will create an e-Consult report. The report will contain information on possible advisory opinion, recommended care and next steps.
Step 5: e-Consult is completed and report sent to submitting provider. Total time for e-Consult process typically is estimated to be completed in 10 business day or less unless otherwise noted. In some cases the timeline may extend beyond 10 business days.
e-Consult disclaimer
The service provided through the e-Consult service is different from the diagnostic services typically provided by a physician. The Children's Specialty Group physicians providing this service will not have the benefit of information that would be obtained by examining the patient, obtaining their own history and observing the patient's physical condition. Therefore, the physician may not be aware of facts or information that would affect his or her opinion of the patient's diagnosis. By deciding to engage this service, you acknowledge and agree that you are aware of this limitation and agree to assume the risk of this limitation.
By requesting an e-Consult through this website, you acknowledge and agree that:
- The advisory opinion and/or recommendation is limited and provisional;
- The consultation is not intended to replace a full medical evaluation with a physician;
- The physician(s) performing the e-Consult does not have the benefit provided by a direct physical examination.
By engaging our services, you acknowledge and agree to solely assume the risk of these limitations, you further understand that no warranty or guarantee has been made concerning any particular advisory opinion, result or cure of the patient's condition.
Use of this service does not oblige the provider to refer to Children's Hospital of Wisconsin, Children's Specialty Group, The Medical College of Wisconsin or any affiliated providers.
