Unplanned Transfers
Why we measure it - In order to ensure we are providing the best and safest care, we track the number of unplanned transfers to a higher level of care. This tracking and continual review of each unplanned transfer gives us the opportunity to identify potential areas of improvement.

What this means - A patient may need to be transferred to a higher level of care when the treatment they are receiving in their current setting is no longer meeting their clinical needs. An unplanned transfer occurs when a patient needs more intense monitoring or care requiring a transfer, which was not anticipated. An unplanned transfer can occur for multiple reasons and is defined broadly. Each unplanned transfer is reviewed to identify potential opportunities for improvement.

What we're doing to provide the best care:
- The medical director reviews each patient who had an unplanned transfer for any improvement opportunities.
- We have the expertise and equipment to handle any transfer within our hospital system and never have to transfer a child to another facility for specialty care.
- As a hospital, we look at patient flow for any trends and to make sure patients are being cared for in the best possible way.
Patients and Families:
- Bring a full list of the medications your child is taking including over-the-counter medicines such as vitamins and supplements.
- Be an advocate for your child. Participate in daily bedside clinical discussions, provide any information about your child that may be helpful to staff.
