Case Management
Children's Service Society of Wisconsin provides case management services in a variety of different programs throughout the state. Through case management, Children's Service Society staff members coordinate and provide services and support to children and their families ensuring that services being provided are responsive to the goals and needs that are identified for the family. The basic goal of case management is to assist families in identifying their needs and providing them with options for services and support.
Examples of programs in which Children's Service Society provides case management services are:
Case Management Assessment & Support Team (CAST) - Marathon County: The goal of CAST is to support clients in their attempts to achieve self-sufficiency and manage the stress of the W-2 process. A case manager, working in conjunction with Marathon County Department of Employment & Training staff, meets with the client to assess his or her needs, develop a plan and support the client in carrying out the plan.
Supervised Visitation - various locations: Provides a location for parents to visit their children in a controlled setting. Trained staff provides either random checks or are with the family at all times during the visit to monitor the interaction between parents and children to assure safety. The child and parent are offered ideas for recreation and play during that time.
Wraparound - Milwaukee: A court ordered program that coordinates mental health services with families that are struggling with an identified child who has a serious emotional, behavior and/or mental illness. Some of the services include, but are not limited to: care coordination (home visits once a week at a minimum), in-home therapy, medication management, out-patient therapy, housing assistance, day treatment, parent aide, respite care, tutoring and mentoring.
Project Home - Kenosha: The goal of Project Home is to improve permanency planning for children and reduce the length of stay in out-of-home care placements. Staff members assist families to meet the conditions set by the court for the return home of their children or to assist in either terminating parental rights or transferring guardianship to a relative. |
