Membership form

Since the founding of Children's Hospital of Wisconsin and Children's Service Society of Wisconsin in the late 1890s, estate gifts have played an important role in our long traditions.

The Guardian Society recognizes our special friends who have made a commitment to Children's Hospital and Health System through a planned gift. It is our way to say "thank you" for their investment in the future of our mission.

As a member of our estate recognition society, you will join others who share your values, provide a legacy for future generations of children, and make a significant difference in the care of children.

Membership

Membership is open to everyone who has arranged a future gift to the health system. The following types of commitments, in any amount, qualify you to join:

  • bequest provision in your will or revocable trust
  • remainder beneficiary designation of a life-income gift, such as a charitable gift annuity or charitable remainder trust
  • beneficiary designation of a savings, brokerage or other financial account
  • beneficiary designation or ownership of a life insurance policy
  • beneficiary designation of a retirement plan asset, such as an IRA, 401(k) or 403(b)
  • remainder interest in your personal residence, retaining the right to live there until you no longer need it

  • charitable lead trust that provides income to the health system for your lifetime or term of years, with the principal remaining part of your estate

Benefits of membership

We have chosen a few simple ways to recognize your valued commitment. As a new member you will receive a membership certificate along with a small gift of appreciation to display in your home or office.

We hope you will also join us for our annual estate donor recognition luncheon where you will have an "insider" look at the health system's valuable work, learning about exciting discoveries and achievements in addition to future plans. And, you will have the opportunity to meet others who share your feelings about the health system.

You will also receive our planned giving publication, The Guardian, with updates and information on the health system along with gift planning information.

In addition, with your permission, we would like to add your name to our honor roll listing in foundation publications. By making your gift known, you will demonstrate a commitment to the hospital, to your community and to children. Others may follow your leadership, increasing the impact of your decision. However, whether you choose to be named as a member or remain anonymous is your preference. We hope you will let us know of your plans.

If you have made a planned gift to the health system, but not previously notified us about your gift, then please register a new membership.

If you are in the process of updating your estate plan, or just beginning to prepare one, we invite you to consider including the health system in your plans. Your gift will indeed make a difference - to the health system, our programs and services, and most of all, to the children.
