Report a patient care concern
If you have a concern about patient care provided at one of our hospitals or clinics, we encourage you to let us know. Your child's care will not be limited due to a complaint.
To report a concern, contact the appropriate administrator:
Children's Hospital of Wisconsin - Milwaukee (hospital or clinics): Diana Vincent, patient representative, (414) 266-2801 or dvincent@chw.org.

If speaking with the administrator fails to provide a resolution or you choose to skip this step, you may file a grievance with the hospital or clinic using the following process:
- Submit your grievance either verbally or in writing.
- The hospital or clinic must specify to you the time frames to investigate and respond.
- The hospital or clinic must provide a written notice of its decision, which includes:
- Name of the hospital or clinic contact person.
- Steps taken on behalf of the patient to investigate the grievance.
- Outline the results of the grievance process.
- The date of its completion.
Grievances regarding quality of care or premature discharge will be reviewed by the responsible leaders on a timely basis.
Children's Hospital of Wisconsin, Children's Hospital of Wisconsin-Fox Valley and Children's Hospital of Wisconsin-Kenosha are accredited by The Joint Commission. If the concerns in question cannot be resolved through the administrator or grievance process, you are encouraged to contact The Joint Commission through their Web site.
