David S. Smith, MD, leads the Down Syndrome Clinic, offering more than 25 years of clinical expertise to patients and families. Dr. Smith also practices at Hartford Clinic, Hartford, Wis., a part of Froedtert and Community Health system. 

Terri Couwenhoven, MS, is the clinic coordinator and health educator. She specializes in developing programs and resources that address health and sexuality issues for people with developmental disabilities. She also supports professionals who care for these individuals and their families. She is the mother of Anna, who has Down syndrome.

Patients and families may refer themselves to the clinic, or they may be referred by their primary care physician or other health care specialist.
