Children's Community Health Plan

Children's Community Health Plan (CCHP) is a health maintenance organization (HMO) for BadgerCare Plus-eligible families living in Brown, Calumet, Kenosha, Milwaukee, Outagamie, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, Waupaca and Winnebago counties in Wisconsin. Children's Community Health Plan is affiliated with Children's Hospital and Health System.

Children's Community Health Plan, owned and managed by Children's Hospital and Health System, is a health maintenance organization dedicated to providing access to the highest quality health care and service to BadgerCare Plus customers. With the combined resources of Children's Hospital of Wisconsin, Children's Service Society of Wisconsin, Children's Medical Group and our community partners, we are committed to improving the medical and social well-being of our members and the entire community.
