Outpatient Radiology Orders

1. Print the Order form by clicking the Printer icon in the Adobe toolbar below the Address or Location box in your browser.  Do not use your browser's Print button.
2. Diagnosis and ordering physician's signature are required on all orders. Incomplete or unsigned orders are not valid and will require resubmission.
3. For non-surgical patients, fax the completed form to (414) 266-2597.  
4. For surgical patients, fax the completed form to (414) 266-3378.
5. Click your browser's Back button to return to this page.
