Children's Hospital of Wisconsin Neurology Program is a consultant service. Our aim is to work with primary care physicians to help provide care for patients close to home.

To refer a patient, please complete and sign the referral form and fax it to (414) 266-1879. In order to view the referral form, you must have Version 4 or higher of Adobe Acrobat Reader installed on your computer. If you cannot view the form, contact Central Scheduling at (414) 607-5280 or toll-free (877) 607-5280.

Neurology specializes in seeing patients presenting with the following conditions: seizures, headaches/migranes (initial consultation), tics, tremors, ataxia, hypotonia, microcephaly, macrocephaly, brachial plexus injury, Tourette Syndrome or Muscular Dystropy.   

 

Neurology does not specialize in the following conditions: ADHD/ADD, autism, behavior problems, behavioral difficulties or problems in school (suboptimal/academic school performance).  Please refer these patients to Child Development or Psychiatry.

Whenever possible, Children's Hospital staff will provide initial evaluations and coordinate with primary care physicians to implement an ongoing treatment plan. Patients with developmental delays and/or attention deficit disorder or behavioral problems will be directed to Developmental Pediatrics and the Child and Adolescent Psychiatry and Behavioral Medicine Center, respectively.
