Notice of privacy breach July – November 2013

Children’s Hospital of Wisconsin had a breach of privacy related to medical records in 2013. Despite numerous attempts, we have been unable to make contact with 20 of these families.

From July to November 2013, an employee who responds electronically to parents requesting an electronic copy of Children’s authorization to release medical records form, on occasion, inadvertently included an internal tracking sheet as an attachment. The tracking sheet contained a list of patients whose medical record information was released in accordance with policy from January through June 2013. The information in the tracking sheet included the medical record number, patient’s first and last name, a description of the information released, and the requestor to whom the information was released.

No other private information was compromised by this incident. No billing or social security numbers were released, so no action is required by these families to protect their identities.

Our records indicate the patient families we have been unable to reach live in the following communities:

  • West Allis, Wis. (1)

We take this issue very seriously. Our staff has received additional training to be sure this type of breach does not happen again.
